The 40-year-old comedian has revealed that she agonised over whether to expand her family - at one point breaking down into tears over the issue.
The 30 Rock star writes in her new memoir, Bossypants, that she felt torn between motherhood and her Hollywood career.
She says her daughter Alice, five, would say 'I wish I had a baby sister', leaving the star feeling 'stricken with guilt and panic'.
In a exert released to America's People magazine, Fey recalls how she would 'debate the second-baby issue' when she couldn't sleep.
'I get up to go to the bathroom and study myself in the mirror,' she writes.
'Do I look like someone who should be pregnant? I look good for 40, but I have the quaggy jawline and hollow cheeks of a mom, not a pregnant lady. It's now or never. This decision can't be delayed.'
The Emmy winner, who is married to composer Jeff Richmond, later turned to her gynaecologist for advice.
'I went for my annual checkup and, tired of carrying this anxiety around, burst into tears the moment she said hello,' she writes.
'I laid it all out for her, and the main thing I took away from our conversation was the kind of simple observation that only an impartial third party can provide: "Either way, everything will be fine."'
She concluded: 'What's so great about work anyway? Work won't visit you when you're old. Work won't drive you to get a mammogram and take you out after for soup.'
Fey revealed she is now five months pregnant during a taping of The Oprah Winfrey Show yesterday.
Fey's 30 Rock cast mate Jane Krakowski is currently pregnant with her first child.
Jane is due to give birth at the end of this month with her British-born clothing designer fiancé Robert Godley.
